Key Responsibilities Service Delivery & Person-Centred Practice Lead the delivery of person-centred supports in accordance with the HSE New Directions framework and organisational policies. Develop, implement, and review individualised programmes and activities that promote community inclusion, personal development, education, recreation, volunteering, and employment opportunities. Ensure each individual has a current Personalised Care and Support Plan that is developed collaboratively and reviewed regularly. Support individuals to achieve meaningful outcomes and maximise independence within their communities. Monitor service effectiveness and adapt supports to meet changing needs. Leadership & Team Management Provide day-to-day leadership, supervision, and support to Support Workers. Foster a positive, accountable, and collaborative team culture. Ensure staffing levels and skill mix meet assessed needs and service requirements. Conduct regular supervision meetings and support staff development and performance management processes. Ensure staff are trained and competent in safeguarding, health and safety, New Directions standards, and organisational policies. Governance, Quality & Compliance Ensure services operate in compliance with all relevant legislation, standards, funding requirements, and organisational policies. Support implementation and ongoing maintenance of the HSE EASI tool and other quality assurance systems. Conduct service audits and quality reviews, identifying opportunities for improvement and implementing corrective actions. Maintain high standards of documentation, record keeping, and reporting. Provide regular updates and reports to the Community Services Manager. Safeguarding & Protection Promote a culture of dignity, respect, and safeguarding across all services. Ensure safeguarding procedures are embedded and consistently implemented. Respond promptly and appropriately to safeguarding concerns and ensure all reporting obligations are met. Support staff understanding and adherence to safeguarding policies and procedures. Health, Safety & Risk Management Promote a positive health, safety, and wellbeing culture. Ensure incidents, accidents, and near misses are reported, reviewed, and managed appropriately. Support risk assessment and risk management processes across services. Manage resources responsibly and in line with allocated budgets. Stakeholder Engagement Develop and maintain positive relationships with individuals supported, families, advocates, community organisations, and multidisciplinary teams. Work collaboratively with the HSE and external stakeholders to deliver high-quality community supports. Communicate effectively with the Community Services Manager regarding service performance, risks, and opportunities for development. Administration & Record Management Maintain accurate, professional, and confidential records. Ensure effective use of electronic systems and reporting platforms. Uphold GDPR and confidentiality requirements at all times. Essential Criteria Experience Minimum 2 years’ experience working within social care, disability services, or a related sector. Experience supporting individuals with intellectual disabilities, Autism, and/or physical or sensory support needs. Experience delivering person-centred supports and programmes. Full clean driving licence and access to own vehicle. Knowledge & Exposure Knowledge of the HSE New Directions Interim Standards for Day Services. Understanding of safeguarding, risk management, and incident management practices. Experience with quality assurance systems, audits, and service improvement processes. Experience working with community groups, families, multidisciplinary teams, and external stakeholders. Qualifications QQI Level 5 qualification in Health and Social Care, Childcare, Pre-Nursing, Youth and Community Work, or a related discipline. Commitment to ongoing professional development. Desirable Criteria QQI Level 7 qualification in Applied Social Studies (Social Care) or a related field. Previous experience in a supervisory, team lead, or co-ordination role. Qualification or accredited training in leadership, supervision, or management. Experience implementing New Directions and person-centred planning frameworks. What We Look For
