Social Care Worker – Primary Care.

Social Care Worker The post is in Primary Care and the person would be working closely with the primary care social workers. This role will cover Dublin South West mainly and be based either in Clondalkin or Tallaght The job description The candidate would support varying minority groups across all age ranges. These include individuals, their families, children’s parents, older persons and vulnerable adults with disabilities. The Social Care Worker would assist people to live safely and independently in the community, under the guidance and direction of the Primary Care Social Work Team. The role is person‑centred, rights‑based and focused on promoting wellbeing, safety, inclusion and advocacy for service users and their families. The Social Care Worker would provide direct practical, social and emotional support to Social Work Clients in the community, implementing care and support plans developed by the Primary Care Social Work Team. They would promote independence, safeguard vulnerable adults and contribute to early intervention and prevention by supporting people to manage difficulties before they escalate into crisis. Requirements of Social Care Worker A minimun of one years experience Must have a CORU registration and number to prove it, submitted candidates await registation will not be accepted. #MEDAHP
