Medforce Healthcare Recruitment is currently seeking a Professionally Qualified Social Worker (P.Q.S.W.) to join a Primary Care service in South Dublin. This is an excellent opportunity for an experienced Social Worker to join a multidisciplinary community-based service supporting individuals and families across a diverse range of needs. The successful candidate will work closely with colleagues across Primary Care and other community services, providing professional social work interventions and contributing to a person-centred approach to care. Key Responsibilities of the Professionally Qualified Social Worker Provide professional Social Work assessment and intervention to service users and families. Manage an allocated caseload in accordance with professional standards and service requirements. Identify and assess social care needs and develop appropriate intervention plans. Work collaboratively with service users, families, carers, and other relevant professionals. Liaise with multidisciplinary teams and external agencies where appropriate. Advocate for service users and support access to appropriate community and social care services. Maintain accurate and up-to-date case records and documentation. Participate in multidisciplinary meetings and contribute to care planning. Work in accordance with relevant legislation, HSE policies, professional standards, and best practice. Contribute to service development and quality improvement initiatives. Requirements for the Professionally Qualified Social Worker Recognised qualification in Social Work. CORU registration as a Social Worker is essential. Previous experience working as a Social Worker within a community, Primary Care, healthcare, or similar setting is desirable. Strong knowledge of professional Social Work practice and relevant legislation. Excellent communication, interpersonal, and organisational skills. Ability to manage a caseload effectively and work independently. Ability to work collaboratively as part of a multidisciplinary team. A full driver’s licence and access to a car may be required depending on service requirements. What We Offer HSE salary scale.
