Speech and Language Therapist – Adults Primary Care The Speech and Language Therapist will provide a comprehensive, high-quality Speech and Language Therapy service to adults within the Primary Care setting. The post holder will assess, diagnose and manage adults presenting with communication, voice, cognitive-communication and swallowing disorders, working collaboratively with individuals, families, carers and the multidisciplinary team to maximise communication, independence, health and quality of life. I would consider a Staff or Senior grade for this position based in Mullingar. Responsibilities of Speech and Language Therapist – Adults Primary Care Provide comprehensive assessment, diagnosis and management of adults presenting with communication and swallowing disorders. Assess and manage clients with acquired communication disorders, including aphasia, dysarthria, apraxia of speech and cognitive-communication difficulties. Assess and manage adults presenting with dysphagia, providing recommendations to optimise safety, nutrition and hydration. Assess and manage voice disorders where appropriate within the Primary Care remit and refer to specialist services when indicated. Develop, implement and regularly review evidence-based intervention plans tailored to individual needs and goals. Deliver intervention through individual therapy, group programmes, education, counselling and consultation as appropriate. Promote self-management and support clients to achieve maximum independence and participation in everyday activities. Provide education and advice to families, carers and healthcare professionals regarding communication and swallowing management. Identify clients requiring referral to specialist or acute services and facilitate timely onward referral. Maintain accurate, comprehensive and timely clinical records in accordance with professional and organisational standards. Participate in multidisciplinary case conferences, care planning meetings and integrated care pathways. Contribute to discharge planning and transition of care where required. Participate in service planning, evaluation, quality improvement initiatives and clinical audit. Assist in the development and review of clinical guidelines, care pathways and service protocols. Monitor clinical outcomes and contribute to service performance indicators. Practice within professional scope and maintain high standards of clinical governance. Engage in continuing professional development and lifelong learning. Participate in clinical supervision, peer review and performance development processes. Attend mandatory training and maintain competence in all required areas. Requirements of Speech and Language Therapist – Adults Primary Care Recognised qualification in Speech and Language Therapy. Registration with CORU or eligibility for registration. Full clean driving licence and access to transport for community visits. Clinical experience in the assessment and management of adult communication and swallowing disorders through undergraduate placements or post-qualification practice. #MEDAHP
